These are questions I've used in my online classes.
Weekly Evaluation
How was the amount of information in the course so far? How useful was it to you?
How was the organization of the information presented in the course so far? Were you able to understand and follow what is expected of you?
How clear was the information presented? Were there enough examples and illustrations?

Final Evaluation
How was the pace of the class? How was the amount of information for each week?
How useful was the information presented?
How were the hands-on activities? Did you get enough practice?
Do you think you can now implement what you learned? If not, what else do you need?
How did the "technology" work for you in the course? Did it help, hinder, or facilitate the learning? What do you think would improve it?
Did you receive sufficient feedback and comments from the instructor(s)? Did you feel they were "present"in the course?
Having gone through the course, what would you have liked to know before you started?
Do you have any other comments to share? (Note that each question is individually anonymous. If you want to write a specific note from you, you can put your name here and the other questions you answer will still be anonymous.)
What stood out to you as the most significant new piece of information in this module? Was there enough time spent on this new concept? Do you feel other teachers would find this particular information helpful and would benefit from a more detailed look at this area?
